After my initial visit, is there anything I need to bring to future appointments?
Yes. You will need to show the receptionist your photo ID and insurance card at every single visit. It is very important that you bring these items.
Will you remind me of my appointment?
As a courtesy, we provide reminders via an automated service. However, we ask that you keep in mind automated systems can fail at times, so please do not count on this. You are ultimately responsible for keeping your appointment.
Our service should deliver a voice call two days in advance, and the menu will give you an option to confirm. All calls are made in the morning hours. Text message reminders are delivered in the evening hours, and cannot be replied to. If you wish to reschedule or cancel, please call our office at (773) 786-9779
during regular business hours (Monday- Thursday 9-5, and Fridays 9-4:30).
Will I be charged if I cancel an appointment?
We ask that you give as much notice as possible if you need to cancel or reschedule an appointment. If you cancel an appointment with less than 24 hours notice, you will be charged a cancellation fee. For new patients, the fee is $100. For existing patients, it is $20.00 to $50.00.
What if I forget to bring my insurance card with me?
We will request that you send us a copy by fax, mail, or email within 24 hours of your appointment. If your insurance information has not been verified, you will need to provide a credit card which will be charged at the time of your appointment. If your insurance is verified within two days, the charge can be reversed. Please be aware that email is not considered a secure communication option, but it is available if you choose to send your information this way.
Is my doctor available by email?
You will have the opportunity to sign up with our patient portal, a convenient resource for managing your medical care online. You can message your provider securely from within the portal. You should receive a response within three working days. Please understand this form of communication is not intended for emergency situations.
How can I sign up for the Patient Portal?
After your first visit, we will send you a pin number that allows you to register. The Patient Portal is a secure platform that gives you easy access to results of recent lab tests, bill payment, secure messaging, prescription refill requests, appointment scheduling, and more. If you are currently a patient and have not signed up for the Patient Portal, you can request your registration pin from the receptionist the next time you visit our office.
Are you accepting new patients?
Yes, we are accepting new patients, and Dr. Perlmuter currently has openings in his coveted concierge program. We thank you in advance for your patience, as first-time appointments typically need to be scheduled several weeks in advance. However, once you are an established patient, if you join the concierge program, you will enjoy same- day or next-day appointment availability.
